Check out some of the military equipment companies for good low profile padded gun belts.

First Spear and HSGI are two excellent manufacturers

As for the belt pouch, I've been using a lindcraft small AC pouch for a long time. Recently, I began to use a chest-mounted pack from Hill People Gear. I work in the rain a lot, and the chest pouch is much easier to wear under or over rain gear- I usually wear it under to keep all my stuff dry.

This is the best AC pouch I've been able to find. It's the perfect size; bigger then the small AC pouches, but not as big as those Jumbo AC pouches. Has a great pocket for you flashlight and can fit all the pens/sharpies/slate markers you need with plenty of room left over. Mine has held up pretty well for the last 2 years. The belt is really comfortable and the padding helps it stay up on your hips.

I recently made the switch from waist pouch to chest pouch and it's made a huge differance. Far more comfortable, more organised and things are a lot more accessible i.. special sections for board marker, torch, leatherman, scissors etc. Pretty much all of the chest pouches in the UK are this model, rarely see a waist pouch anymore (at least for 2nd AC's).

The only recommendation I'd make is adding more Velcro for where the lid sticks, I stitched in strip of 2" Velcro because with the default design I noticed the lid come open a couple of times when the pouch was full and had items fall out, a strip of 2" Velcro fixed this completely and had no issues since.
