Explain camera report forms please!

Audio is the sound mixer's job. No reason for it to appear on a camera report, other than to note if a shot is MOS.

 

 

 

If thats the case how does the editor know what audio clip goes with what.

audio people have their own paperwork.

normally they give us that paperwork at end of day, and it goes along with camera reports to the lab (so some time after wrap transport will be asking camera assistants 'how long until film and sound is ready?')

A camera report form is listed on this post. Do you know where I can find a good form for the audio side of things? Or at least whats it called?
