Camera Rental/Rental Form

Hi, I am workign on a feature and am renting out my camera package on the show here in Los Angeles. Does anyone know of or can recommend a camera contract or rental form? Thanks very much in advance.

I'm not sure what you mean. You generally talk to the rental person and create a list of what you want, get a bid, go down to prep the package for pick-up, and get a final rental list to sign, plus the rental agreement that they type up for you -- you don't provide the agreement or form. At some point, I'm sure there will also be talk of proof of insurance. But in terms of filling out the paperwork, you generally do that on the day you come to pick-up the package unless they fax it to you in advance to fill-out.
