Canon FD to Bayonet Mount Adapter

So I want to be able to use my Canon FD lenses on an Arriflex 16SR, what adapter would I need for that?

The FD's flange focal depth is shorter than the Arri's, so you can't do it without a correction lens. (42 vs. 52mm).
